Obedience and Rally Entry Form Instructions



Having Trouble with the Form? The PDF reader programs do not work properly in some browsers, making our fillable form unusable.  DOWNLOAD the form directly from the link and try:
  • Mac users: Open the form using "Preview", a free PDF reader program installed on Macs.
  • Mac and Windows users: Go to Adobe to download Acrobat Reader free for your OS.
  • OR print the form, fill it out manually and mail it or email the scanned form to the Show Secretary.

Our new fillable PDF form will automatically calculate your Entry Fee and provides a link to our secure Credit Card processing system. Filling out the form on your computer will make it more legible and accurate.

  1. Click on the link "2024 VA Derby Entry Form" to open the form in a new browser tab.
  2. Fill out the complete form, one entry per Dog, the form will accommodate both Obedience and Rally entries for all three days.
  3. You must choose the option to pay by credit card or by check.
  4. PLEASE remember to save the completely filled-out form to your device. Your typed name will serve as your signature.
To Pay By Credit Card
  1. Click on "Pay by CC" link. If link does not work in your PDF reader, use this link. To open the secure payment page in a new tab, press the Ctrl button and click the link
  2. Enter the total credit card amount calculated in the form and fill out the information of the card holder.
  3. Send the completed form you saved and send it as an attachment to "show@dcbouvier.org". Receipt of your email will be your entry time. Your credit card will be processed by "Stripe" and you will receive a payment receipt by email from them.
To Pay by Check
  1. Complete the fillable form, click on "Pay By Check", save and print it.
  2. Make Checks payable to GWBdFC for the amount shown on your entry form.
  3. Then fill out your check and mail it with your printed form to:
